Two questions about the rules since I don't see them in there.
First, it's said that the top two teams by score of (3, 1, 0; W, T, L) make it to the finals.
Is the two lower QR ranked teams also going to be scheduled to play for 3rd and 4th place, respectively.
Second question,
How are ties broken in the event of a tied score?
- In regards to how I think you will answer that question, what if the two teams tied in question are 2-1-2 against each other?

I was thinking about a 3rd and 4th place match, but I doubt the teams would have motivation to show for a consolation match.
a Tie will be broke by who won the head to head match.
I highly doubt there is going to be two teams tied in points and tied each other in the head-to-head match as well, but I will come up with a 2nd tie breaker as well.

I don't see why they wouldn't want to play myth if they signed up to play, unless they really got sick of their draft team after the first 3 weeks.
You should schedule it anyway.
Also,
You have a CTF pick every week,
but week 2 has an assassin map;
this kinda weights the 3 matches differently, and can cause problems with your 'head-to-head tie breaker' resolution.
you should maybe even move the assassin map to the finals and take a map from the finals that is non-tieable in order to provide fairness throughout the scoring possibilities of the QR weeks.
Back to the matter at hand,
if the teams are 2-1-2 in h2h and pts, how would you resolve that tie breaker ? technically, this is only a problem if the teams are tied at 2 (rather than 1 or 4th). you could, in that case, put up maps for a 3 or 5 game tie breaker / semi-finals match; in case it were to happen.
